These documents, which are based on the department of health and human services format, summarize important information about each health plan option so that consumers can easily compare Kaiser Permanentes` services and coverage with those of other airlines. Kaiser Permanente looks forward to working with you on the distribution of SBCs. We will provide applicants with the appropriate SBC as soon as we receive their application. We provide SBCs that you can provide to your customers upon request. You can deploy SBCs in paper or electronic form.
